Custom anchor doesn't work with jekyll-commonmark

https://arrow.apache.org/visual_identity/ uses ## ... {#XXX} for custom anchor. It's available with Kramdown but not available with jekyll-commonmark.

I didn't see this when I filed #696 but that's an immediate fix for this. Does that work for you @kou?
