Breaking (v0): Remove PostgreSQLCatalogTypeName. Use FormatPostgreSQLType for PostgreSQL-dialect type strings (nil → empty string).

Documentation: TypeCode_STRUCT formatting is documented as provisional; output may change when PostgreSQL-dialect support for composite types is clarified.
