• Add pure Go PDF text extractor for memory ingestion (no external dependencies)
• Handles FlateDecode/zlib compression, Tj/TJ operators, hex strings
• Detects word spacing from TJ array negative numbers
• Add application/pdf to default memory ingest types

Streaming Tool Output Fixes

• Fix streaming format to match apteva-kit/debug UI expectations
• Use unified type: "tool_stream" with event field (chunk/output/progress/log)
• Enable streaming for parallel tool execution - streaming tools now stream even when multiple run simultaneously
• Each stream includes tool_id for client to distinguish sources
