Fix: Thread activity summarizer accuracy

  • Messages now include chronological numbering (#1, #2, etc.) so the LLM can identify ordering
  • Added footer indicating which message is the most recent
  • Updated prompt to explicitly instruct focusing on the LATEST exchange only for activity
  • Earlier messages are used only for generating the thread title

This fixes the bug where activity would repeat an older action (e.g. "Notification sent") even after a newer exchange.
