Fix: register connect_operator_session in all tool whitelists.

The tool was created in v1.52.0 but wasn't added to:

  • config.go auto-add lists (2 places)
  • main.go operator tool names
  • handlers/config/handler.go cleanup list
  • tools/test_mode.go simulation

Now the LLM sees connect_operator_session when operator mode is activated.
