v4.0.0 - Major Modernization for hapi v21+

Breaking Changes

• Node.js >= 18.0.0 required
• @hapi/hapi >= v21.0.0 required
• Plugin now uses modern exports.plugin format with async/await
• Handlers must use response toolkit (h) instead of reply
