v1.29.3 — skill path fix and README corrections

Extension 1.29.3 / mcp-server 1.29.3 — stale skill path.

Fixed

• The sketch-plane warning pointed at a file that no longer exists. It told
  agents to read skill/SKILL.md "Pen axis mapping" before sketching; the skill
  moved to skills/shapeitup/SKILL.md. The warning, two source comments and the
  README's install command now use the new path.

Extension 1.29.2 / mcp-server 1.29.2 — documentation only; no code changes.

Fixed

• README quick example used an API that doesn't exist. It called
  holes.through(plate, "M3", [[x, y, z], …]); holes.through(size, opts)
  returns a cutter and takes no plate or positions. The example now cuts the
  holes with patterns.cutAt(plate, () => holes.through(...), patterns.grid(...))
  and was checked with verify_shape and render_preview.
• Tool counts. Both READMEs said 25 MCP tools and SKILL.md said 20; the
  server registers 29. The tool lists and SKILL.md's tool table now include
  open_viewer, close_viewer, check_stack, run_simulation,
  clear_params, describe_geometry, validate_joints, verify_shape and
  sweep_check where they were missing.
• Export formats. The README listed OBJ; export_shape accepts STEP, STL
  and 3MF. SKILL.md listed only STEP and STL.
• Changelog pointer. packages/mcp-server/CHANGELOG.md stopped at 1.16.3
  with no hint of where later releases went; it now points here.
