OpenCode agents + known-list-only detection

• OpenCode agents now read correctly: agents and rules load from ~/.config/opencode (markdown agents in agents/), and the config resolves opencode.json/opencode.jsonc automatically.
• Only the known tool list is scanned: the home-directory heuristic auto-scan is now opt-in (Settings → Detection). Unrelated folders like Jenkins, Gradle, etc. no longer show up as agents.
• Missing agents/plugins/sessions folders render as an empty directory instead of a 404.
