Zero-friction: surface the no-key path at every first touch

• npx zero-install entry documented (README.md): npx loki-mode tour works
  today with no global install, no provider CLI, no API key, no spend - the
  lowest-friction taste of the verified-completion receipt. It is now the leading
  "Get Started" line (it was previously undocumented).
• CLI first-touch screens lead with loki tour (autonomy/loki): the no-args
  landing and the first-run welcome both previously gated all value behind a
  provider CLI + API key (doctor/quickstart/start); they now offer loki tour
  first - a real sample result with no key and no spend - so a brand-new user
  sees value before any setup.
