Loki Mode v7.42.0

Ten-team parallel hardening + documentation completeness wave (worktree-isolated,
file-sharded, merged after independent review). Closes the remaining deferred
bug-hunt findings and documents the v7.41.x default-on knobs across all surfaces.

Fixed

• MCP LSP proxy no longer stalls the event loop: every tool handler offloads its
  blocking LSP I/O via anyio.to_thread, with per-request-id and stdin-write
  locking added since the handlers are now truly concurrent. lsp_get_diagnostics
  also sends textDocument/didChange when a file changed on disk, so it returns
  fresh diagnostics instead of the first-open snapshot.
• Dashboard app-runner liveness now detects PID reuse: a recycled PID whose
  process start time is later than the recorded launch is treated as stopped,
  instead of reporting a dead run as "running" forever.
• Healing hooks (currently unwired) made correct-when-wired:
  hook_post_healing_modify reverts only the healing edit via a pre-edit snapshot
  (no longer git-checkout-nukes unrelated uncommitted changes, and reports
  honestly for untracked files); hook_pre_healing_modify uses path-aware matching
  instead of substring containment.
• Completion council contrarian transcript fields (contrarian_triggered /
  contrarian_flipped) are recorded correctly from a pre-decrement snapshot
  (audit-trail only; the live vote was already correct).
• Admin commands (loki failover, projects, enterprise token) pass user values to
  their python helpers via environment variables instead of raw string
  interpolation, so a value containing a quote no longer silently no-ops while
  the file goes unwritten.
• Bun route captures LOKI_CAVEMAN_USER_MODE from the inherited
  CAVEMAN_DEFAULT_MODE at startup, so the caveman no-raise / opt-out guard is
  live on the Bun route (was dead; dormant until the Phase 6 sunset).

Added

• Documented the v7.41.x default-on opt-out knobs
  (LOKI_REVIEW_INCONCLUSIVE_BLOCK, LOKI_COMPLETION_TEST_CAPTURE, LOKI_AUTO_DOCS,
  and the caveman compressor knobs) across wiki (Environment-Variables,
  Quality-Gates), README, and docs/INSTALLATION, with each default verified
  against source. Synced references/ and skills/ docs with the v7.41.x
  verification-integrity behavior. Fixed a stale README claim that Claude Fable
  was an available premium API tier (it collapses to Opus everywhere).
