QTI 3.0 dialect for the QTI adapter (engine#158)

learn-content-engine/qti spoke QTI 2.x only. 1EdTech's current version,
QTI 3.0, keeps the semantics of the mappable subset (choice, text entry,
match) and renames the syntax: qti- prefixed kebab-case elements
(qti-choice-interaction) and kebab-case attributes (response-identifier)
under the imsqtiasi_v3p0 namespace. The adapter now reads both dialects,
detected from the root element (importQti is unchanged for 2.x callers and
refuses a root that is neither), and writes either: exportQti(lesson)
stays byte-identical 2.x, exportQti(lesson, { version: "3.0" }) emits 3.0.
Same mapping table, same loud refusal list (issues report the canonical 2.x
interaction name), same fidelity limits, same round-trip guarantee. The
dialect difference lives in three pure functions in src/qti/dialect.ts;
the 3.0 fixtures follow the element and attribute spellings of 1EdTech's
implementation guide (checked verbatim for the choice example). Widening
the mapped subset stays a separate decision (docs/qti.md non-goals).
