docs: correct route domain index (#5434)

Summary

• correct the mounted digital-twin identity and autobiography prefixes
• distinguish Apple Health routes from system health checks
• remove the duplicate workspace-context entry
• add remote desktop and rapid reader route domains

Test plan

• verified every documented prefix against server/index.js
• verified /api/workspace-contexts appears exactly once
• ran git diff --check
• completed Codex and Claude local review with no findings
