:boom: ESM explicit import pattern — all APIs from "sugg", zero globals

BREAKING: Runtime APIs (exec, execFile, scanPath, readFile,
readJson, ui, cache, fetch) must now be imported via
import { ... } from "sugg" — no more global injection.

- createCompletion and dynamic remain built-in
- fetch API with configurable timeout
- AGENTS.md template for AI tooling
- Tests: 57/57 passing
