  3. short_report — Deep research reports < 300 chars
  4. reasoning_leak — Internal reasoning leaked as final response
  5. tool_failure_chain — ≥ 2 failures in last 3 tool calls
  6. api_fault — Any HTTP error, timeout, or network failure
• Saves session snapshots to llm-wiki/feedback/{ts}-{signal}.json
• 20-event ring buffer + auto-purge (keeps latest 50 files)
• Fire-and-forget — never blocks the agent

Fixed an issue where the Wiki Browser file tree would crash on fresh installations.

Fix 修复

• listDir crash on fresh install: nsIFile.initWithPath throws NS_ERROR_FILE_UNRECOGNIZED_PATH when wiki directories don't exist yet. Now wrapped in try-catch, returning empty tree instead of crashing.
