Would it be possible to distribute the extension with the flatpak and not make it a different distribution path? I know that gnome-extensions tends to lag behind releases quite some time occasionally.

I am not sure, but ofc there are multiple applications that have their top bar icon similar to a shell extension (easyeffect for example)

Yes, it seems that most apps choose to depend on the "AppIndicator and KStatusNotiierItem Support" gnome-shell extension to show their top bar UI. At least that's what Easyeffects seems to be doing and a couple of others I checked.
