Replaces the binary "config set / not set" model of host health monitoring with an explicit three-state selection, so callers can distinguish "no preference" from "deliberately off". Previously the only way to influence monitoring was to attach (or omit) a HostHealthConfig; there was no way to explicitly turn the host heath monitoring off versus simply leaving the decision to an enclosing configuration layer.
