Since v1.0:
* fixes WAV input via pipe in hdcd-detect
* sample rates other than 44100 are handled better
* bit depths other than 16 can be scanned, but not decoded
* fixes possible integer overflow in cdt calculation
* fixes PE table bounds check in _hdcd_envelope()
