New command: /dev:review

Two-phase PR review that combines deep local analysis with automated comment resolution.

Phase 1: Code Review

• Runs pr-review-toolkit:review-pr agents (code, errors, tests, types, etc.)
• Produces structured report: Critical → Important → Suggestions → Strengths
• --post flag additionally runs code-review and posts results to GitHub

Phase 2: Address Open Comments

• Fetches all open PR comments: inline review comments, top-level discussion, and bot reviews
• Detects bot authors (CodeRabbit, Copilot, SonarCloud, etc.) vs human reviewers
• For each actionable comment: makes the code fix, replies to the thread, or notes won't-fix
• Asks for confirmation before committing and pushing
