v1.2.4 - Remove duplicate hooks reference

🐛 Bug Fix

Fixed duplicate hooks loading error by removing redundant hooks reference from plugin.json.

What Changed

• Removed "hooks": "./hooks/hooks.json" from plugin.json
• The standard hooks/hooks.json is loaded automatically by Claude Code
• The manifest.hooks field should only reference additional hook files

Understanding

Claude Code automatically loads hooks/hooks.json from the standard location. The "hooks" field in plugin.json should only reference additional hook files beyond the standard one.
