Fix undef safety and add page type support

• Add 'page' type support in addition to 'entry' in edit_entry_param
• Fix potential undef warning on $app->param('_type') with // ''
• Fix potential undef warning on $param->{html_head} with //= ''
• Update description and README to mention both entry and page editing screens
• Bump version to 1.0.2
