• 新增：宿主机渠道支持 ask_user_question 提问通知——模型调用 ask_user_question 时，系统通知与飞书/钉钉/企业微信/通用 Webhook 会发送「等待回答」提醒。
• 修复：浏览器原生通知因固定 tag 导致连续审批/提问只显示首条的问题。
• 变更：README 安装固定版本更新至 #v1.2.0，并补充 tool/call 触发条件与更新日志。

• Added: host channels now support ask_user_question notifications — when the model calls ask_user_question, the system channel and Feishu/DingTalk/WeCom/generic webhooks send a "waiting for answer" reminder.
• Fixed: browser native notifications only showed the first pending approval/question because of a fixed notification tag.
• Changed: README install pins updated to #v1.2.0, and trigger conditions / changelog updated.
