Network timeouts everywhere in the agent hot path

• Every fetch an agent can trigger now has a hard timeout. Previously the 6
  embedding-provider calls (OpenAI/Gemini/Mistral/Cohere/Ollama/cachly) and several
  vector-store calls had no AbortSignal — a slow or unreachable provider could
  hang the entire agent turn.
• Added AbortSignal.timeout: embeddings 8 s (CACHLY_EMBED_TIMEOUT_MS), semantic
  search + all cache.ts/context.ts/brain.ts/tco.ts vector calls 8 s,
  apiFetch already had 15 s. The CLI health check is now bounded too.
• A memory tool must never block the agent: embedding failures already degrade
  to keyword-only recall; now they also fail fast.
