  physical source URL while preserving the 15-minute request floor across

Fixed

• Commands now reject ignored positional arguments and subcommand-inapplicable
  options instead of appearing to accept them.
• Custom API and hosted MCP URLs must be valid HTTP(S) URLs, and MCP rejects a
  hosted-only --url when local mode would otherwise ignore it.
• Retained compatible deprecated client request types for launch stability,
  removed the unused --discover flag and duplicate contracts, and ensured MCP
  subprocesses receive the caller-injected runtime environment.
