docs: confirm Codex SessionStart dogfood

What changed

• Update the Codex support matrix to record that recall-at-start is verified.
• Replace the stale “verifying” caveat with the exact live validation status for agentcairn 0.24.2 / Codex plugin 0.1.2.
• Keep SessionEnd wording precise: its installed handler is probe-tested and capture remains backed by cairn sweep.

Why

After PR #130 and release v0.24.2, a fully restarted Codex session successfully executed the corrected SessionStart command, injected AgentCairn’s savings/recent-memory context, and showed no exit-127 failure. Live MCP recall and remember/recall round trips also passed against a current, healthy vault index.

Validation

• Fresh Codex SessionStart injection observed in the rollout log with no adjacent failure event
• Live MCP recall succeeded (freshness: current)
• Live MCP remember was written/indexed and immediately recalled
• cairn doctor --vault /Users/ccf/agentcairn: status: OK
• git diff --check

The agents table now states recall-at-start is verified (replacing the prior “verifying” ambient-hooks wording). The [^codex-hooks] footnote records end-to-end verification of the plugin, bundled MCP server, and SessionStart recall hook, and clarifies SessionEnd: same corrected command-string dispatch, handler probe passes, with cairn sweep still noted as the out-of-band capture backstop.

No code, config, or release artifacts change—README only.
