bugfix: card-mod compatibility

• Fixed card-mod styling support by keeping a stable top-level ha-card wrapper.
• Fixed an intermittent responsive layout race where icons could disappear during dashboard reloads when early width measurements incorrectly classified rows as compressed.
• Improved test coverage for card-mod compatibility, stable ha-card rendering, and responsive density behavior.
