Follow-up to v6.41.5 / ADR-137. The Text-view TOC showed raw inline-markdown for headings containing links (e.g. Security (CSE) · [13](iris://diagram/…)); heading text is now flattened to plain text. Rendered headings also gained slug ids matching the TOC, so clicking an entry now scrolls to the heading. Versions 6.41.5 → 6.41.6.
