Fix episode list scrolling to top on episode check

Checking off an episode further down in a long episode list jumped the
whole card back to the top. _render() fully rebuilds the .body element
containing the episode list on every re-render, which reset its scroll
position to 0 each time.

- Preserve .body scrollTop across re-renders
- When marking an episode (and all earlier ones) watched, auto-collapse
  earlier seasons that just became fully watched as a result (matching
  existing behavior when first opening a show), and explicitly scroll to
  keep the just-checked episode in view instead of losing it behind the
  now-collapsed sections
- Bump CARD_VERSION (13 -> 14) and manifest version (2.0.1 -> 2.0.2) for
  cache-busting, update CHANGELOG and README example URLs accordingly
