fix: sequential op calls to reduce biometric prompts to 1

Problem: Promise.allSettled fired N parallel `op item get` calls
simultaneously, each triggering a separate 1Password biometric prompt.

Fix:
- Add listVaultItems() for a single `op item list` call upfront
  (warms up biometric session + discovers existing items)
- Sequential `op item get` only for providers that have items
  (reuses established session, 0 extra prompts)
- Skip op calls entirely for providers not yet in vault
