Refactor ContainerAdapter and update documentation

Refactored `ContainerAdapter` to use instance fields for `IServiceProvider` and `IServiceCollection`, improving encapsulation and thread safety. Updated `ServiceCollectionExtensions` and `ServiceProviderExtensions` to use the new instance-based `ContainerAdapter`. Removed `ContainerAdapterContainer` and consolidated its functionality into `ContainerRegistration`. Added `IgnoreContainerRegistrationAttribute` to exclude classes from automatic service registration. Enhanced README.md with detailed explanations, usage examples, and new sections on custom IoC container adapters and architectural concerns. Added a new unit test to verify behavior of ignored attributes.
