• Remember how each note opens (new setting, on by default): a note converted with
  Open as mindmap opens as a mindmap next time; Open as note restores it, in the Reading /
  Editing mode it had when converted. The record lives only in the plugin's own data.json and
  never touches the note; it follows renames and moves and is cleaned up on delete.
• Two kinds of map, told apart: the ribbon icon and the command palette open a following map
  that tracks the active note; Open as mindmap produces a pinned map that belongs to one note,
  stays put when other notes are opened, and is still pinned after a restart.
• Fixed: the map kept showing the previous note. When switching notes the map now reads the
  file from disk instead of an editor that has not loaded the new note yet, and follow decisions
  use the file and tab carried by the event, so clicking the map itself can no longer switch it away.
• From the file explorer, Open as mindmap converts the tab that already shows the note instead
  of opening a second one.
• Switching form (note ⇄ map) does not enter tab history; Back and Forward move between notes only.
• 9 new unit tests, 524 in total.

• 记住每篇笔记的打开方式（新设置，默认开）：用「打开为导图」转换过的笔记，
  下次打开直接就是导图；「打开为笔记」即恢复，并还原转换时的阅读 / 编辑模式。
  记录只存在插件自己的 data.json 里，笔记本身一个字节都不变；改名、移动、删除时自动迁移或清理。
• 两种导图分清了：侧边栏图标与命令面板打开的是跟随型导图，跟着活动笔记走；
  「打开为导图」换出来的是钉住的导图，只属于那一篇，再打开别的笔记它也不动，重启后仍然钉着。
• 修复导图总是显示上一篇笔记：切换笔记时改为直接读取磁盘上的内容，不再在编辑器尚未装入新笔记的空隙里读到旧正文；
  跟随判定改用事件自带的文件与标签页，点一下导图自己不会再把图切走。
• 从文件列表右键「打开为导图」时，这篇若已经开着就就地转换那个标签页，不再另开一个。
• 形态切换（笔记 ⇄ 导图）不进入标签页历史，「后退」「前进」只在笔记之间移动。
• 新增 9 项单元测试，共 524 项。
