Reviewed project upgrades

• Adds explicit /ctx404 upgrade planning and approval.
• Supports the first reviewed migration: �0.2.0-beta.1 → �0.3.0-beta.1.
• Uses a local backup, preserves current state/topics/project definitions, validates with doctor and rolls back on failure.
• Ordinary /ctx404 remains idempotent and never silently overlays a planted project.

Validation

• 13 deterministic tests passing.
• Landing verified on desktop and a 390 px mobile viewport without console errors or horizontal overflow.
