v0.4.3 — Persist error in state for failed flows

Fix: failed flows now show why they failed

InstanceRecord now includes an error field. All failure paths (env resolution, node execution) persist the error message to the state file. flow_status can now report the actual error instead of just status: "failed" with an empty trace.
