Fixed

• Hybrid idle timeout stops over-logging idle-but-open sessions (src/usage-history/daily.ts):
  the usage tracking idle timeout now uses a hybrid approach that distinguishes
  between truly idle sessions and sessions that are open but not actively
  exchanging tokens. Previously, sessions that were open (e.g. waiting on a
  long generation) were incorrectly logged as idle time, inflating the idle
  minute count. The fix gates idle detection on concurrent_sessions === 0
  so that open sessions continue to count as active even without token movement.

• Dashboard empty state centering and perf tab layout (dashboard/): the
  empty state placeholder is now properly centered, the performance tab layout
  is tightened, and usage tooltip separators render correctly across all
  viewport sizes.

• Reference docs reorganized — documentation files under docs/ have been
  reorganized for clearer navigation. AGENTS.md tightened with consolidated
  config field table. .github/AGENT_RULES.md added for AI agent behavioral
  rules (thinking stamping, reasoning_effort stamping rules).

• Sleep/wake gap test — new integration test covering the usage tracking
  behavior across system sleep/wake gaps, ensuring that gaps in sampling data
  (e.g. laptop sleep) are handled correctly without spurious idle time.
