Fixed

• Vision cache miss now rewrites in foreground: processBodyCacheOnly
  previously fire-and-forgot to background vision on a cache miss, forwarding
  the original image-bearing body unchanged. Non-vision models would then
  receive an image they cannot process. On a cache miss, the proxy now halts,
  calls the vision model, rewrites the body with the text description, then
  forwards. Subsequent requests for the same image hit the cache and skip the
  vision call. Removed enqueueBackgroundVision and its regression test.

• Vision call request_body/request_headers now persisted: the
  updateVision SQL and VisionUpdateParams interface gained
  $reqBody/$reqHeaders/$reqSize parameters. VisionImageProcessor
  populates them, enabling dashboard inspection of the exact vision request
  including intent context (adjacent text, system prompt excerpt).

Added

• Catalog-strategy integration tests: three end-to-end tests covering
  Bug 1 (non-vision model gets rewritten text, not image), Bug 2 (vision
  request_body persisted with intent context), and vision-capable model
  passthrough (no vision call).
