Multi-harness Phase 2b — skills speak each harness's tool vocabulary.

• Per-harness skill vocabulary. install-skill (and auto-heal resync) now rewrite each skill's
  tool names for the target harness: a Grok install gets monitor / run_terminal_command /
  ask_user_question and /loop 60s, while Claude keeps Monitor / Bash / AskUserQuestion and
  /loop 45s. So a Grok agent's skills reference the tools Grok actually has. (Grok's allowed-tools
  is guidance, not a sandbox, so the confer-arm no-shell guarantee is enforced by confer's runtime
  backgrounded-watch check rather than the frontmatter.)
