Multi-harness Phase 4 — the safety kernel + re-arm nudges reach every runtime.

• session-heal now writes its context to ~/.confer/session-context.md — the safety kernel,
  fleet roster, and any re-arm nudges — a harness-agnostic delivery alongside Claude's SessionStart
  additionalContext stdout. Runtimes that ignore SessionStart stdout (Grok Build) pick up the context
  by reading the file; the /confer-watch skill's first step now does exactly that. The file is
  rewritten on every heal and cleared when there's nothing to say, so it never goes stale.
