Add TF1+, with TF1, TFX, TF1 Séries Films and LCI

TF1 streams its channels on TF1+. telesfor now offers TF1, TFX and
TF1 Séries Films, which are free with a TF1+ account, and LCI, which
needs none, from a tuner of their own at /tf1, numbered from 5001.
TMC is left out: TF1+ streams it DRM-protected.

Since v0.2.7, a stream that comes in more than one quality is also
played in the best one the connection keeps up with, and segments
are fetched over HTTP/1.1 so an abandoned fetch holds up no other.
