fix: generate textgen titles for forked sessions - #251

Use only post-fork user input when generating automatic session titles so copied source history cannot make the guard reject the mechanical fork title. Include the Claude terminal reason and result in missing structured-output errors to expose authentication failures. Validation: bun run check (614 tests)
