docs(plugin): define reconstructible operation identity - #262

Document that reconcile operation refs and ordering must be reconstructed deterministically from durable Resource state or stable re-observable facts. Update the example and public type comments so plugin authors do not derive continuation identity from process memory, randomness, or current time.
