fix: show pending Codex steers in queue - #276

Keep an accepted steer in the existing composer queue until Codex emits the correlated userMessage receipt, then project it into the transcript. Distinguish current-turn steers from next-turn follow-ups and document the two-phase delivery lifecycle. Tests: bun run typecheck; bun test tests (617 passed) Note: specgen:check is currently blocked by unrelated ignored .worktrees entries.
