Sync inline approval mode changes

Summary

• add an adapter event for approval mode changes made inside inline PTY prompts
• persist that event into the daemon session summary and broadcast state without writing transcript rows
• emit the event from zarvis interactive approval prompts when a / f changes the mode
• treat the event as hidden/no-op in TUI display paths and document the rule
