This PR refactors tool calls for both aether/wisp's MCP clients and its 1st party MCP servers to use the new MCP MRTR protocol.
This is a large refactor that sheds a significant amount of complexity from the codebase. Previously MCP servers could send elicitation requests at any time, which forced single-threaded clients to multi-plex concurrent messages. MRTR is a stateless, sequential response/request cycle that's simpler.
Note that the new MCP tasks extension isn't supported by these changes.
