  required lines ("Ladder: used ... | skipped ... - reason", "Experts: N dispatched / none -
  reason"), and review convergence explicitly has no small-fix exemption — the report's
  convergence line now names each round with its applied count, so a final round that applied a
  finding is self-evidently non-converged.
