Added

ctx import --all --incremental runs one catch-up using existing history checkpoints. It leaves indexing mode unchanged; normal imports remain exhaustive.

Fixed

Foreground imports and setup keep tracking the original request after lost worker replies instead of giving up after three retries. Cancellation and explicit failures remain intact.
