format() completeness — get_study_record and get_study_results

  • clinicaltrials_get_study_record renders leaves structuredContent already carried but format() dropped, including the full observational-study subtree (#18)
  • clinicaltrials_get_study_results full mode walks the complete classes -> categories -> measurements tree instead of sampling the first class/category (#63)
