eSearch reliability and notice fixes, plus a prompts/list argument fix

  • Upstream eSearch failures now throw instead of masking as a zero-hit success; pubmed_search_articles' offset is capped at 9998 to match PubMed's retstart ceiling (#95)
  • pubmed_search_articles surfaces ignored field tags and unmatched phrases from eSearch's ErrorList/WarningList via notice, even on hit-bearing results (#96)
  • Partial dateRange and the summaryCount cap message are now disclosed instead of silently dropped or pointing at a maxed-out knob (#97)
  • research_plan's includeAgentPrompts is now advertised as optional in prompts/list instead of required (#98)
