pubmed_fetch_fulltext retrieval fallback chain fixes

  • EPMC query builders now match: unquoted EXT_ID/PMCID tokens, dropped SRC:PMC filter (#85)
  • front-matter-only PMC/EPMC records fall through the chain instead of being dropped, with a new no-body outcome (#86)
  • pmcids input now resolves a DOI and reaches the Unpaywall fallback tier (#88)
  • Unpaywall articles carry a pmcId backlink so pmcids-input batches key consistently (#92)
  • Europe PMC and OpenAlex clients: dead 404-handling branches removed, fixing a service-error misclassification (#90)
