serverInfo title correction

Fixed:

  • createApp() title corrected to pubmed-mcp-server (was 'PubMed MCP Server' introduced in 2.9.2). MCP clients display serverInfo.title in their UI — this aligns the displayed name with the machine name convention.

847 tests pass; bun run devcheck clean.
