Skipped #67 thinking it was Phase 5+ work (when it's actually Phase 4 follow-up gated only on cron data)
Missed the 7 newly-filed issues entirely

Small-scope but high-leverage: AGENTS.md is the cross-tool agent SoT for "Open Phase 4+ issues", and stale pointers cause repeated re-derivation across sessions.
