Remove frame stagnation detection — superseded by token-rate ceiling

The stagnation guard (detecting when recent tokens attend well behind
the frontier) was added in a6720c2 to catch repetition loops. The
token-rate ceiling + smart reset now handles this more directly: instead
of detecting stagnation after 16 tokens and discarding them, it caps
token production at 15/cycle and resets the pipeline with untranscribed
audio.

3/3 full slow-test runs pass with identical scores after removal,
confirming the guard was redundant with the rate ceiling.
